Well, I've now lost my 2nd GoPro from my R1200GSA. The first one was lost when the attaching screw somehow backed out and it fell off somewhere along Old Ore Road in Big Bend TX. The second one was lost somewhere along the Sam Rayburn Parkway in Dallas when the standard GoPro mounting tab's (the one bonded to helmets of flat areas) flanges broke allowing the GoPro to fall off. This one bothered me most as it was a failure I didn't expect. Be sure to check these mounts for cracked flanges frequently. Here is a YouTube video showing the issue:

Yep, that’ll happen. I use a backup safety cord to ensure that if a mount fails, the camera will still be caught with the safety leash. There are several types to choose from; here’s one example:

Don't know the cost of those, but I use fishing leaders with swivels on all my stuff including phone.
